Adoptive Cell Therapy using CD4 T Helper 1-like and CD8 Cytotoxic T Lymphocytes in a Mouse Model of Melanoma | Adoptive cell therapy (ACT) with tumour-reactive T cells has been shown to be one of the most effective cancer treatments. However, there are many consensus questions that need to be answered so that ACT can deliver a more effective cancer treatment. By using a murine model of melanoma, we attempt to elucidate several strategies to augment the anti-tumour responses mediated by ACT. The traditional ACT focuses only on using the classical killer cell - CD8 cytotoxic T lymphocyte (CTL). CD4 T helper (Th) cells have been gradually shown to be crucial for enhancing CD8 CTL response and inducing complete tumour regression. However, the difficulties in obtaining adequate numbers of CD4 Th cells with desired phenotype through in vitro expansion limits the application of CD4 Th cells in ACT. By using mice with transgenic CD4 Th cells, the methodology for antigen-specific expansion of murine CD4 Th1-like cells in tissue culture has been optimised in this study. This optimal cell expansion utilises 1 ng/ml IL-2 and 5 ng/ml IL-7 and the CD4 Th cells were cultured in advanced-DMEM/F12 medium. Greater than 10,000 fold expansion can be generated in 20-day period with this method; and the majority of these cells were found to express an early effector cell phenotype as CD27+/CD28+/CD44+/CD62L-/IL-7Rg+/PD-1-/CTLA-4-. These CD4 Th1-like cells were not generated under the Th1 polarised conditions, but expressed multiple Th1- cytokines and Th cell transcriptional factors. Cytolytic activity was also identified in these CD4 Th1-like cells, suggesting they may be a unique subset of cytotoxic Th cells. These CD4 Th1-like cells were found to be able to persist in the lymph nodes and spleen of the naiΜve recipient mice and remained functionally active for at least 30 days, without challenge with the target antigen. In addition, both in vitro expanded CD4 and CD8 CTL were able to persist for at least 150 days in mice challenged with subcutaneous tumours that expressed the target antigens To investigate the factors associated with the efficacy of ACT, a series of ACT with these in vitro expanded CD4 Th1-like and CD8 CTL, with or without Dacarbazine and VLP-OVA vaccine treatments, was performed in a subcutaneous mouse model of melanoma. The VLP-OVA vaccine is derived from Rabbit Haemorrhagic disease virus (RHDV) Virus-like particles that express Ovalbumin. By themselves, CD4 Th1-like cells were not as good as CD8 CTL in controlling tumour growth. However, co-transfer of CD4 Th1-like cells and CD8 CTL significantly enhanced tumour regression, leading to complete cure in 80% of the mice. A combination of ACT and VLP-OVA vaccine treatment induced similar anti-tumour response to the primary B16-OVA tumour challenge. However, this combination treatments enhanced memory and endogenous immune response to tumour re-challenge with a mixture of B16-OVA and B16-gp33 cells. Addition of the Dacarbazine treatment to the ACT did not enhance immune response to the primary B16-OVA tumour or the generation of an endogenous immune response to the tumour re-challenge with B16-gp33 cells. However, DTIC was found to synergise with VLP-OVA vaccine to delay the growth of B16-OVA tumour. These data clearly demonstrate the ability of these in vitro expanded CD4 Th1-like cells in enhancing tumour regression mediated by CD8 CTL, and inducing durable memory response to tumour recurrence. Two Heads Are Better Than One: The Combination of Dendritic Cells & B Cells Yields An Improved T Cell Response Over Dendritic Cells Alone When Presenting Tumour Lysate Antigens | BACKGROUND The best APC for tumour immunotherapy is yet to be defined. The dendritic cell (DC) is well established as the key professional antigen-presenting cell (APC) for priming naΓ―ve T cells against tumours and for this reason it is the APC of choice in adoptive cell therapy (ACT) for cancer. However, robust clinical results with DC ACT remain disappointing and researchers are beginning to investigate the utility of alternative APCs, particularly B cells. DCs have been used as vaccines clinically to present tumour antigens in the form of tumour cell lysates, also with limited success. Tumour cell lysates represent an attractive source of tumour-associated antigens (TAAs) for cancer immunotherapy as their defined and undefined TAAs have the potential for simultaneous activation of CD4+ and CD8+ cytotoxic T lymphocytes (CTLs). Although their self-antigenic nature requires overcoming of inherent tolerance mechanisms as well as caution with regards to induction of autoimmunity. METHODS In this study the ability of three professional APCs, DCs, macrophages (MΞ¦s) and B cells, to stimulate an anti-tumour T cell response against tumour lysate (TL) antigens was compared. The activation profile and IL-12 response of the APCs to soluble freeze-thaw lysate (s-L) and oxidized whole freeze-thaw lysate (ox-L) was examined. Also investigated was the lysate-loaded APCsβ ability to induce T cell proliferation and effector functions, whether individually or in combination. RESULTS The three APCs displayed differing responses to the two TLs indicative of differing T cell activation capacities. MHC-II and CD40 were upregulated on DCs in response to s-L and ox-L while MHC-II and CD86 were upregulated on B cells in response to s-L and ox-L. A synergistic increase in IL-12 production was observed in the lysate-loaded DC+B cell groups. No proliferation response was observed in lysate-primed CD4+ T cells. However, significant increases in CD8+ T cell proliferation were observed when T cells were primed with combinations of s-L-loaded APCs compared to a DC alone. The greatest proliferation was observed when CD8+ T cells were primed by the ox-L-loaded DC+B cell combination. The greatest IFN-Ξ³ levels were also observed in CD8+ T cells primed with lysate-loaded DC+B cells. The greatest in vivo cytotoxicity was achieved by ox-L-primed CD8+ T cells activated by the DC+B cell combination. SIGNIFICANCE Both DCs and B cells are currently being investigated as APCs in adoptive cell therapy for cancer. This study demonstrates that combining DCs and B cells yields an improved CD8+ T cell response in a pre-clinical model of melanoma. Tumour lysate-based immunotherapy for colorectal cancer | In New Zealand, the incidence and mortality rates of colorectal cancer (CRC) are among the highest worldwide. Despite drastic advances in our understanding of this disease, current treatments for metastatic CRC remain largely ineffective, highlighting the urgent need for more effective therapies. Recently, therapeutic strategies that harness anti-tumour immune responses β called immunotherapies β have achieved unsurpassed clinical efficacy in a subset of cancers. Given the crucial role of the immune system in controlling CRC progression and metastasis, immunotherapies represent a promising approach for the treatment of this disease. However, despite their tremendous potential, technical constraints currently limit the clinical application of cancer immunotherapies for solid tumours such as CRC. Most notably, the identification of targetable tumour antigens remains a highly expensive, difficult, and laborious process. Consequently, simplified approaches to immunotherapy that circumvent the need to identify specific tumour antigens are attractive alternatives, offering both clinical efficacy and broad applicability. Tumour lysates provide a source of both undefined tumour antigens and endogenous adjuvants β called danger-associated molecular patterns (DAMPs) β to induce anti-tumour immune responses. Tumour lysates can therefore serve clinical applications for immunotherapy β including vaccination and adoptive cell therapy β without having to identify specific tumour antigens. Utilising the MC38 murine CRC model, this thesis investigated the feasibility of using tumour lysates as a simplified but broadly applicable immunotherapy for CRC. In the first study, longitudinal immune phenotyping revealed dynamic changes in T cell infiltration during progressive MC38 tumour outgrowth. Important immune-suppressive mechanisms identified in this model include the upregulation of inhibitory immune molecules, the presence of deleterious or dysfunctional immune populations, and the physical exclusion of T cells from the tumour parenchyma. Importantly, these mechanisms reflect those identified in human CRC patients that contribute to immunotherapy resistance. Consequently, MC38 represents a clinically relevant model for investigating immunotherapies β especially human CRC subtypes that exhibit similar biological mechanism(s) of immune evasion. In the second study, methodology to improve the therapeutic efficacy of tumour lysates was investigated. Specifically, the exposure of tumour cells to either oxidation (using hypochlorous acid) or heat shock (at 42Β°C and 56Β°C) were evaluated as strategies to enhance tumour lysate immunogenicity. The immune-stimulatory properties of different tumour lysate preparations were then compared in vitro and in vivo. Despite poor correlates of immunogenicity in vitro, prophylactic vaccination with tumour lysates could elicit protective anti-tumour immunity in vivo. However, the results of this study provided little evidence that the induction of cell stress could enhance the immune-stimulatory properties of tumour lysate preparations. Importantly, findings from this chapter advocate for the careful characterisation and optimisation of methodology for tumour lysate preparation, as well as careful selection of in vitro and in vivo models to assess their immunogenicity. In the third study, tumour lysate-loaded antigen-presenting cells were investigated as a platform for the expansion of tumour-specific T cells. In a model antigen system, lysates prepared from tumour cells exposed to severe heat shock could facilitate the ex vivo expansion of antigen-specific CD8+ T cells. These T cells retained desirable phenotypic and functional attributes for use in adoptive immunotherapy but failed to mediate robust tumour regression when transferred into tumour-bearing hosts. This failure was attributable to mechanisms of immune-suppression intrinsic to the MC38 model, highlighting the need to target active immune-resistance mechanisms to potentiate the efficacy of immunotherapies. In the fourth and final study, a potential mechanism underlying the immunogenicity of lysates prepared from severe heat-shocked tumour cells was investigated. Results from this chapter offer evidence that the degradation of antigen by stress-induced proteases can enhance subsequent cross-presentation in vitro. Important mediators implicated in this phenomenon include tumour-derived calpains and the association of antigenic peptides with necrotic tumour cell debris. Together, these findings suggest that stress-induced proteases can increase the availability of antigenic substrates for cross-presentation, thereby enhancing the immunogenic potential of tumour cell death. Importantly, this data extends the links between cell death and immunity and may have profound implications in the contexts of infectious disease and autoimmunity, in addition to cancer immunotherapy. In conclusion, this thesis demonstrates that immune-suppressive mechanisms active in CRC tumours represent a prominent barrier to effective immunotherapy and highlights inherent limitations to the use of tumour lysates as a therapeutic approach. Thus, while simplified approaches to immunotherapy may offer the advantage of widespread applicability, more sophisticated approaches are needed to realise the therapeutic potential of immunotherapies in the context of CRC. In this regard, personalised combination strategies that are tailored to individual patientsβ specific tumour microenvironments hold immense promise for the next generation of cancer immunotherapies. Opening the gates : factors influencing general practitioner referral to osteopathy | Traditionally General Practitioners (GPs) have referred patients to physiotherapy for manual therapy. In 1988 only 15% of Wellington GPs referred patients for osteopathic treatment (Hadley, 1988). Since then there has been an increase in the popularity of complementary and alternative medicines, including osteopathy. The osteopathic profession in New Zealand has recently undergone several changes. These include regulation under the Health Practitioners Competency Assurance Act 2003 and expansion as a profession with the first new practitioners graduating from a Masters approved New Zealand based training programme. On the eve of osteopathic registration, Auckland based GPs were surveyed regarding their referral of patients to osteopathy, physiotherapy and chiropractic, the factors that influence their referral patterns, and their attitudes towards osteopathy. Results were compared to a similar survey completed 10 years earlier. It was found that physiotherapy is still the first choice for GP referral to manual therapy, although referrals to osteopathy have increased. There also appears to have been a shift away from chiropractic and towards osteopathy. Although GPs demonstrated attitudes of medical dominance and superiority over alternative therapies, it appears that positive treatment results experienced by patients influence GP referral patterns. The need for scientific evidence establishing the effectiveness of osteopathic treatment was highlighted. GPs claimed to have knowledge of osteopathy yet demonstrated a limited understanding with the confined scope of practice for which they would refer to osteopathy. Hidden hunger in a land of plenty: Food insecurity in Aotearoa New Zealand | Aotearoa New Zealand is a relatively wealthy, food-producing nation. Yet, increasing numbers of the populace are experiencing food insecurity, hidden hunger, and material hardship. More than an individual issue, ensuring food security requires consideration of the social and cultural components of growing, preparing, and consuming food and meals. Access to sufficient food to eat and the surrounding social practices are central to human flourishing. This article documents three research exemplars that challenge dominant global representations of Aotearoa New Zealand as an idyllic haven. Each case explores the invisibility and downplaying of hunger and poverty that occur within this βland of plentyβ. In describing and reflecting our respective research projects we illustrate how broader socio-political and economic factors play out materially in the everyday lives of marginalised groups by restricting access to food. Do Γ la carte menus serve infertility patients? The ethics and regulation of in vitro fertility add-ons. | Add-on treatments are the new black. They are provided (most frequently, sold) to patients undergoing in vitro fertilization on the premise that they will improve the chances of having a baby. However, the regulation of add-ons is consistently minimal, meaning that they are introduced into routine practice before they have been shown to improve the live birth rate. Debate on the adequacy of this light-touch approach rages. Defenders argue that demands for a rigorous approval process are paternalistic, as this would delay access to promising treatments. Critics respond that promising treatments may turn out to have adverse effects on patients and their offspring, contradicting the clinician's responsibility to do no harm. Some add-ons, including earlier versions of preimplantation genetic testing for aneuploidy, might even reduce the live birth rate, raising the prospect of desperate patients paying more to worsen their chances. Informed consent represents a solution in principle, but in practice there is a clear tension between impartial information and direct-to-consumer advertising. Because the effects of a treatment cannot be known until it has been robustly evaluated, we argue that strong evidence should be required before add-ons are introduced to the clinic.
